TL;DR — Preceding fever is specific but insensitive for septic arthritis in acute monoarthritis; rapidly destructive hip OA can destroy the joint within 6 months to 3 years, and previous intra-articular injection is a recognized septic-arthritis risk factor (Kano 2025, PMID 41138310; Chau 2024, PMID 38936340; Naik 2025, PMID 39836892).

Infection after injection: rare, not zero

Of 14,118 intra-articular glucocorticoid injections (and 8,252 arthrocenteses) in one centre, 11 septic arthritis cases occurred within 14 days — 0.08% (95% CI 0.03–0.12); male sex, age and pre-existing joint disease were risk markers (Petersen 2019, PMID 31146626). In a US propensity-matched cohort, septic-arthritis risk after large-joint corticosteroid injection was 0.18%; the matched risk difference versus no injection was −0.07% (95% CI −0.09% to −0.05%), so the study did not find increased risk under current practice (Cooper 2025, PMID 40607839). Previous injection is nevertheless a recognized septic-arthritis risk factor in clinical review and septic arthritis requires prompt diagnosis (Naik 2025, PMID 39836892). THA after recent hip injection is associated with higher postoperative infection risk in a timing-dependent way (Werner 2016, PMID 26803711). Preceding fever/rigor starting ≥1 day before joint symptoms was specific (96.8%) but insensitive (27.8%) for septic arthritis in acute monoarthritis — absence of fever does not exclude infection (Kano 2025, PMID 41138310).

Rapidly progressive / destructive OA

Rapidly destructive hip OA can progress to complete joint destruction in 6 months to 3 years (Chau 2024, PMID 38936340). Meta-analytic incidence of rapidly progressive OA after hip corticosteroid injection was 6% (3–9%) across 10 studies able to detect it, with heterogeneous definitions and possible selection bias (Sabatini 2023, PMID 37941925). NGF-inhibitor programs made RPOA a formal joint-safety endpoint: composite events 38.3 and 71.5 per 1,000 patient-years on tanezumab 2.5 and 5 mg vs 14.8 on NSAIDs (Hochberg 2021, PMID 33538113); 145/4,541 (3.2%) in pooled SC studies, dose-dependent, 0% on placebo (Carrino 2023, PMID 37652258). Repeated intra-articular triamcinolone every 12 weeks for 2 years increased cartilage loss without pain benefit (McAlindon 2017, PMID 28510679) — a structure signal, not RPOA by the tanezumab adjudication definition.

NSAID vascular and GI harm

CNT: coxibs major vascular RR 1.37 (1.14–1.66); diclofenac 1.41 (1.12–1.78); naproxen 0.93 (0.69–1.27); ~3 extra major vascular events per 1,000 patient-years on coxib or diclofenac (CNT Collaboration 2013, PMID 23726390). Real-world IPD (446,763 people, 61,460 MIs): 1–7 days of NSAID use already raised MI odds, including naproxen 1.53 (1.07–2.33) (Bally 2017, PMID 28487435). da Costa 2021: opioid clinical benefit did not outweigh harm; 83.3% of opioid regimens increased AE withdrawal (da Costa 2021, PMID 34642179). SPACE: opioids not superior for function, more medication-related symptoms (Krebs 2018, PMID 29509867).

NICE atypical-feature list (recent trauma, prolonged morning stiffness, rapid worsening, hot swollen joint, infection/malignancy concern) is a differential prompt, not a diagnostic rule (NG226, accessed 2026-08-31).
