TL;DR — Exercise reduces knee- and hip-OA pain, but meta-analytic rankings do not establish a universally superior mode; in IDEA, diet plus exercise improved pain and function more than exercise alone in older adults with overweight or obesity (Liang 2025, PMID 41124830; Mo 2023, PMID 37346776; Messier 2013, PMID 24065013).

Exercise: sufficient evidence of a small average effect, not a universal winner¶
Trial sequential analysis concluded that by 2002 enough evidence existed that exercise beats no-exercise control for lower-limb OA; a combined strengthening + flexibility + aerobic package improved function with SMD −0.63 (95% CrI −1.16 to −0.10) (Uthman 2013, PMID 24055922). Fransen's Cochrane review: land exercise improved knee pain 12/100 (10–15) immediately and 6/100 (3–9) at 2–6 months after formal treatment; function 10 then 3 points; only 35% of 54 trials met three core bias safeguards (Fransen 2015, PMID 26405113). Lawford's 2024 Cochrane update: pain 8.70/100 better immediately (5.70–11.70; 28 studies, 2,873 people; low-certainty) and function 11.27 (7.64–15.09; moderate-certainty); compared with MIDs of 12 pain / 13 function / 15 QoL points, CIs either missed the MID or included both important and unimportant effects, leaving clinical importance uncertain (Lawford 2024, PMID 39625083).
Holden IPD meta-analysis: exercise vs non-exercise reduced pain by small amounts that shrank over time; people with higher baseline pain and worse function benefited more, especially at 12 weeks (Holden 2023, PMID 38251550). Yan 2025 network: aerobic exercise had the highest probability of being best across pain, function, gait and QoL (mean SUCRA 0.72) with moderate certainty; strengthening and mixed programs also improved mid-term function (Yan 2025, PMID 41093618). Weng 2023: exercise vs oral NSAIDs/paracetamol, no difference at ~4, 8 or 24 weeks for pain or function (152 RCTs, 17,431 people) — similar efficacy, very different safety (Weng 2023, PMID 36593092). PRT vs NEMEX is a hip-OA null (Kjeldsen 2024, PMID 38588540).
Nationwide implementation, not just RCTs: in the uncontrolled GLA:D registry, 9,825 Danish participants receiving education plus supervised neuromuscular exercise reported mean pain reductions of 12.4 points at 3 months and 13.7 at 12 months (0–100), with QoL increases of 5.4 and 9.4 points; without a comparator, these are implementation outcomes rather than causal treatment effects (Skou 2017, PMID 28173795).
Weight: 5% is a threshold, 10% is better in trials, structure is unproven¶
Christensen 2007 (4 RCTs, 454 people): pooled pain ES 0.20 (0 to 0.39) and disability ES 0.23 (0.04–0.42) at 6.1 kg (4.7–7.6) loss; disability improved when loss exceeded 5.1% or 0.24%/week (Christensen 2007, PMID 17204567). IDEA (n=454, 88% complete): diet+exercise −10.6 kg (11.4%) vs exercise −1.8 kg; WOMAC pain difference 1.02/20 (0.33–1.71) and function 4.29/68 (2.07–6.50) vs exercise; diet lowered compressive force 200 N vs exercise (Messier 2013, PMID 24065013; NCT00381290 COMPLETED). STEP 9 (n=407, BMI 40.3, WOMAC pain 70.9): semaglutide 2.4 mg weekly, weight −13.7% vs −3.2%, WOMAC pain −41.7 vs −27.5 at 68 weeks (Bliddal 2024, PMID 39476339; NCT05064735 COMPLETED). NICE: any loss helps, 10% likely better than 5% (NG226, accessed 2026-08-31). Neither IDEA nor STEP 9 establishes cartilage preservation.
