Hip osteoarthritis¶
TL;DR — Pain with internal hip rotation and radiographic osteophytes or joint-space narrowing support diagnosis; exercise, education, weight loss when appropriate and selected NSAIDs are core care, while advanced symptomatic structural disease may be treated with joint replacement (Katz 2021, PMID 33560326). Hip dysplasia is an OA precursor in which altered mechanics contribute to degeneration (Gala 2016, PMID 26738905).

Morphology that actually raises incident OA¶
Ganz framed femoroacetabular impingement as a cause of hip OA (Ganz 2003, PMID 14646708). Prospective Rotterdam data (n=4,438 without baseline OA; mean 9.2 years) quantify the claim: cam deformity OR 2.11 (95% CI 1.55–2.87) and acetabular dysplasia OR 2.19 (1.50–3.21) for incident radiographic OA; pincer deformity did not increase risk; both cam and dysplasia associations were driven by younger participants (Saberi Hosnijeh 2017, PMID 27696746). Hip dysplasia remains a mechanical precursor treated with periacetabular osteotomy before advanced arthrosis (Gala 2016, PMID 26738905). Lifetime symptomatic hip OA is 25.3% (21.3–29.3) by age 85 in Johnston County, similar by sex and race — so racial THA disparities are not explained by disease occurrence (Murphy 2010, PMID 20713163).
Conservative care is not one exercise recipe¶
Kjeldsen cluster-randomized 160 people with clinical hip OA to 12 weeks of progressive resistance training or neuromuscular exercise: 30-second chair-stand change was 1.5 stands in both arms (difference 0.0, 95% CI −0.8 to 0.8); HOOS pain 8.6 vs 9.3 points (difference −0.7, −5.3 to 4.0) (Kjeldsen 2024, PMID 38588540). PRT is not superior to NEMEX for that functional endpoint. Network evidence for exercise vs oral NSAIDs/paracetamol at 4, 8 and 24 weeks is compatible with no difference (Weng 2023, PMID 36593092).
When surgery-eligible, THR is not equivalent to more training¶
Frydendal randomized 109 people ≥50 years with severe hip OA and a surgical indication: Oxford Hip Score rose 15.9 points after THR vs 4.5 after resistance training (difference 11.4, 95% CI 8.9–14.0) at 6 months. 9% assigned THR had not had surgery; 21% assigned training had undergone THR. Serious adverse events were similar and mostly known THR complications (Frydendal 2024, PMID 39476341; NCT04070027, ACTIVE_NOT_RECRUITING at 2026-08-31 query). This is incremental benefit in already surgery-indicated disease, not a reason to skip education/exercise/weight care.
Posterior-approach hip precautions: pooled RCTs RR 1.8 (0.6–5.2) for early dislocation vs minimal/no precautions; NRS RR 0.9 (0.3–2.5); 8,835 participants; low-certainty — current evidence does not support routine precautions (Korfitsen 2023, PMID 37039064).
