Disease-modifying therapies¶
TL;DR — Sprifermin increased femorotibial cartilage thickness at two years without significantly improving WOMAC scores, while metformin remains supported mainly by preclinical and limited human evidence that still requires confirmatory clinical trials (Hochberg 2019, PMID 31593273; Lim 2022, PMID 35597372).

Structure without symptoms, symptoms without safe structure¶
Sprifermin (rhFGF18). FORWARD (NCT01919164 COMPLETED): 100 µg every 6 months increased 2-year femorotibial cartilage thickness 0.05 mm (95% CI 0.03–0.07) vs placebo; 100 µg every 12 months 0.04 mm (0.02–0.06); 30 µg doses not significant; no significant WOMAC difference at any dose (Hochberg 2019, PMID 31593273). Five-year follow-up (378/549 completers): the 0.05 mm difference for 100 µg q6mo vs placebo persisted (0.00–0.10; p=0.015), while WOMAC pain improved about 50% in every group, including placebo (Eckstein 2021, PMID 33962962). FGF18 is a genetically nominated target (Tachmazidou 2019, PMID 30664745), but the structural result has not established patient-important symptom benefit.
Lorecivivint (CLK2/DYRK1A; Wnt modulator). Phase 2 primary WOMAC pain week 13 was not met; all arms including placebo improved ≥20 points (Yazici 2020, PMID 32432388). Phase 2b found 0.07 mg and 0.23 mg improved Pain NRS vs placebo at weeks 12 and 24 (e.g. 0.07 mg week 24 −0.70, 95% CI −1.34 to −0.06) (Yazici 2021, PMID 33588087). Phase 3 OA-11 (n=513): Pain NRS week 12 LOR −2.24 vs placebo −2.49 (p=0.185); no medial JSW loss in either arm over 52 weeks (Yazici 2025, PMID 39808286). OA-10 (n=498) also missed the primary; a post-hoc KL 2 subgroup showed a week-4 signal (Yazici 2025, PMID 39808288). NCT04931667 (3-year open-label) was TERMINATED; NCT04385303 (STRIDES-1) COMPLETED.
NGF inhibition (tanezumab). Lane 2010: walking-pain reductions 45–62% vs 22% placebo across 10–200 µg/kg (Lane 2010, PMID 20942668). Schnitzer 2019: WOMAC Pain least-squares difference vs placebo −0.60 (2.5 mg) and −0.73 (2.5/5 mg) at week 16 (Schnitzer 2019, PMID 31265100; NCT02697773 COMPLETED). Versus continued NSAIDs, composite joint-safety events were 38.3 (95% CI 28.0–52.5) and 71.5 (56.7–90.2) per 1,000 patient-years at 2.5 and 5 mg vs 14.8 (8.9–24.6) on NSAIDs (Hochberg 2021, PMID 33538113). Pooled SC studies: 145/4,541 (3.2%) adjudicated composite joint-safety endpoint; 0% placebo, 3.2% 2.5 mg, 6.2% 5 mg, 1.5% NSAID (Carrino 2023, PMID 37652258). Analgesia uncoupled from joint protection is the unresolved safety problem (OQ-7).
Bone-targeted and anti-inflammatory attempts. MIV-711 (cathepsin K): NRS pain −1.4 / −1.7 / −1.5 (placebo / 100 / 200 mg) — not significant — while slowing medial femoral bone-area progression (p=0.002 and 0.004) (Conaghan 2020, PMID 31887743). Doxycycline slowed medial joint-space narrowing (0.15±0.42 vs 0.24±0.54 mm in one report of the trial) without reducing pain (Brandt 2005, PMID 15986343). Low-dose methotrexate in knee OA with effusion-synovitis (n=215; NCT03815448 COMPLETED): VAS pain difference 0.3 mm (−6.7 to 7.3) and effusion-synovitis area 0.1 cm² (−0.8 to 1.0) at 52 weeks — null (Zhu 2025, PMID 40455462). Metformin remains preclinical-plus-limited-human (Lim 2022, PMID 35597372).
