Classification criteria are tools, not gold standards¶
ACR 1986 knee criteria were built as classification trees against RA and other painful-knee comparators, not as a community diagnostic rule (Altman 1986, PMID 3741515). ACR 1991 hip criteria: clinical tree sensitivity 86% / specificity 75%; combining pain with osteophytes, joint-space narrowing and ESR <20 mm/h reached 89% / 91% (Altman 1991, PMID 2025304). Kellgren–Lawrence remains the default radiographic grade despite known inter-observer limits (Kellgren 1957, PMID 13498604).
EULAR 2010 assembled a diagnostic ladder from three symptoms (persistent pain, limited morning stiffness, reduced function) and three signs (crepitus, restricted movement, bony enlargement). Assuming 12.5% background prevalence at age ≥45, the estimated probability of radiographic knee OA rose to 99% when all six were present (Zhang 2010, PMID 19762361). That Bayesian construction is not a substitute for an independent gold standard.
NICE NG226 (2022) diagnoses clinically without imaging in people ≥45 years with activity-related joint pain and morning stiffness absent or ≤30 minutes, and does not routinely image unless atypical features suggest another diagnosis (NICE NG226, accessed 2026-08-31). That is a rule-in pathway for typical primary-care presentations, not a research classification.
Why the three clinical sets do not agree¶
In 13,459 GLA:D primary-care participants, 48% met EULAR, 52% ACR and 89% NICE clinical criteria; only 39% met all three (Skou 2020, PMID 31539621). In CHECK, EULAR had the highest specificity and NICE the highest sensitivity against expert “clinically relevant OA”; the three sets agreed on OA vs non-OA for only 67–74% of knees depending on how many follow-up visits had to match (Wang 2024, PMID 37933434). These operating differences matter when a study prioritizes specificity versus sensitivity; neither comparison validates one set as a universal diagnostic gold standard.
Imaging is not a pain assay¶
Among 710 Framingham adults aged >50 with Kellgren–Lawrence grade 0 knees, MRI showed “any abnormality” in 89%: osteophytes 74%, cartilage damage 69%, bone marrow lesions 52%. Prevalence of any pathology was 90–97% in painful knees and 86–88% in painless knees (Guermazi 2012, PMID 22932918). MRI lesions in radiographically “normal” knees are therefore the population default, not a diagnosis. Hannan documented the complementary NHANES discordance: only 47% of radiographic stage 2–4 knee OA reported pain, and only 15% of people with knee pain had radiographic stage 2–4 OA (Hannan 2000, PMID 10852280).
Phenotypes exist as variable clusters, not treatment algorithms¶
A systematic review supported six recurring phenotype families — chronic pain/central mechanisms, inflammatory, metabolic syndrome, bone/cartilage metabolism, mechanical overload, and minimal joint disease — but these were derived retrospectively (Dell'Isola 2016, PMID 27733199). They are hypotheses for enrichment trials, not current care pathways.
