NICE NG226: do not offer arthroscopic lavage or debridement (accessed 2026-08-31). MeTeOR's ITT null with 30% crossover is the honest clinical reading: many patients improve with PT, a minority later choose surgery, and assignment to immediate APM is not superior on average.

Replacement vs partial replacement vs osteotomy

TOPKAT (n=528; 88% 5-year OKS) found no significant Oxford Knee Score difference between partial and total knee replacement at 5 years (1.04, 95% CI −0.42 to 2.50) (Beard 2020, PMID 32369436). UKA vs TKA meta-analysis: shorter stay in trials (−1.20 days) but higher 5-year revision (RR 5.95 in trials, 2.50 in registries) (Wilson 2019, PMID 30792179). In Skou 2015, TKR plus nonsurgical care produced a KOOS4 advantage of 15.8 points (10.0–21.5) at 12 months, with 24 versus 6 serious adverse events and 26% crossover to TKR from nonsurgical care (Skou 2015, PMID 26488691). In 51 adults 18–65 with medial-compartment OA, high tibial osteotomy reduced pain more than a valgus unloader brace at 12 months (Stam 2025, PMID 39832288) — a selected, younger phenotype.

Mean TKA dissatisfaction in one systematic review was 10%, lower than the often-cited 20% (DeFrance 2023, PMID 36252743). Price's Lancet review identifies incomplete pain relief after technically successful replacement as an important outcome gap (Price 2018, PMID 30496082).
