TL;DR — GBD estimated 595 million people with OA in 2020 (7.6% of the global population) and projected 2020–2050 case increases of 74.9% for knee, 48.6% for hand, 78.6% for hip and 95.1% for other OA (GBD 2021 Osteoarthritis Collaborators 2023, PMID 37675071).

Quantified burden, not adjectives

GBD 2021 remains the global denominator: 595 million people (95% UI 535–656 million) in 2020, 7.6% (6.8–8.4) of the world population, with 132.2% (130.3–134.1) growth in case counts since 1990 and high BMI attributing 20.4% (UI −1.7 to 36.6) of the burden (GBD 2021 Osteoarthritis Collaborators 2023, PMID 37675071). Site-specific 2020–2050 case growth was knee +74.9% (59.4–89.9), hand +48.6% (35.9–67.1), hip +78.6% (57.7–105.3) and other OA +95.1% (68.1–135.0) (PMID 37675071). These are modeled symptomatic-radiographic estimates, not a census.

The previous GBD cycle is not interchangeable. GBD 2017 reported an age-standardized point prevalence of 3,754.2 per 100,000 (95% UI 3,389.4–4,187.6) and incidence 181.2 (162.6–202.4), with YLD rate 118.8 (59.5–236.2) — each about 8–10% above 1990 (Safiri 2020, PMID 32398285). GBD 2010 ranked hip/knee OA 11th for global disability, with age-standardized prevalence 3.8% (3.6–4.1) for knee and 0.85% (0.74–1.02) for hip (Cross 2014, PMID 24553908). Do not average 2010, 2017 and 2021 figures: case definitions, severity weights and geography expanded.

Wallace's doubling after age and BMI control is the key controversy on "OA is just aging and obesity": additional postindustrial risk factors remain unidentified (Wallace 2017, PMID 28808025). Murphy found no BMI gradient in lifetime hip-OA risk despite other literature linking obesity to hip OA and replacement — an unresolved site-specific conflict (Murphy 2010, PMID 20713163).

Risk magnitudes that change denominators

Dose-response meta-analysis of BMI vs knee OA: RR 1.59 (1.34–1.81) at BMI 25, 3.55 (2.51–5.11) at 30, 7.45 (4.19–13.13) at 35 versus BMI 22.5 (Zhou 2014, PMID 24990315). Occupational loading OR 1.61 (1.45–1.78; I²=83.6%) overall (McWilliams 2011, PMID 21382500). ACL injury OR 6.81 (5.70–8.13) for later OA; reconstruction does not restore the uninjured baseline (OR 7.7, 6.05–9.79) (Webster 2022, PMID 33852440). Pain–radiograph discordance is 15–76% (pain given radiographic OA) and 15–81% the other way depending on sample and threshold — these ranges must not be averaged (Bedson 2008, PMID 18764949).
