VA/DoD 2026: additional current comparator

The June 2026 VA/DoD hip/knee guideline was identified during the October sweep; it predates the publication window. Its provider summary reviews evidence through July 2025 and replaces the 2020 version. Table 1 differs from the ACR summary on several questions: it takes no position for or against knee viscosupplementation, knee/hip PRP, or glucosamine/chondroitin supplements because evidence is insufficient. This is not an endorsement. Oral NSAIDs or acetaminophen and knee duloxetine receive weak recommendations for; initiating opioids, including tramadol, receives a weak recommendation against (VA/DoD — “The Non-Surgical Management of Hip & Knee Osteoarthritis,” Provider Summary, Table 1, pp. 6–8, accessed 2026-10-22).

Official-source refresh. ACR still lists its 2026 summary at project step 8. OARSI's guideline page now resolves and lists the 2019 nonsurgical document; NICE NG226 still displays a last-review date of 19 October 2022. The AAOS knee page continues to list the 2021 third edition. These are point-in-time website checks, not proof that no other document exists (issuing-body pages, accessed 2026-10-22; see registry).

ACR 2026 update (summary only, posted 2026-09-14)

The ACR posted a four-page summary of its 2026 update of the 2019 ACR/AF guideline on 2026-09-14; the ACR Board approved it on 2026-09-08 and the full manuscript "will be submitted" to Arthritis & Rheumatology and Arthritis Care & Research (American College of Rheumatology — "2026 Update of the American College of Rheumatology Recommendations for the Management of Osteoarthritis of the Knee, Hip, and Hand", https://assets.contentstack.io/v3/assets/bltee37abb6b278ab2c/bltb3d12c34020da842/oa-guideline-summary-2026.pdf, accessed 2026-10-15). On 2026-10-15 PubMed returned no record of it, so this section rests on the summary table alone: strength (strong / conditional / good-practice statement) and certainty are given, but the evidence profiles and effect sizes behind them are not yet public. The 2019 column in the table above is kept as the record of the 2019 document.

Bold rows are the only ones the summary itself flags: three changes of direction since 2019 (massage, methotrexate for erosive hand OA, glucosamine/chondroitin) and one new recommendation (GLP-1 RA). The summary does not say which 2019 recommendations changed in strength only. Weak opioids are not listed separately from opioids. What this does to the cross-guideline picture: on GLP-1 RA the ACR names an incretin drug for OA-associated obesity, which NICE NG226 does not and which no other document profiled in this page's tables does; on acupuncture and hyaluronan the ACR–NICE disagreement persists; and on methotrexate the ACR splits by joint — conditional against for knee/hip, conditional for erosive hand OA. The summary does not show the evidence behind the knee/hip position, so it cannot be read as adjudicating between the modestly positive unselected UK trial and the null synovitis-enriched trial (see disease-modifying therapies; OQ-23).

AAOS corrections, 2026-10-15. JAAOS published errata to the AAOS knee nonarthroplasty third-edition summary and to the AAOS hip OA guideline summary (Brophy 2026, PMID 42825585; Hannon 2026, PMID 42825586); Crossref now lists both as correction updates, not retractions, to the original records (Brophy 2022, PMID 35383651; Hannon 2024, PMID 38781351). The erratum text was not retrievable from PubMed (no abstract), so what was corrected is unknown here; the AAOS columns above should be rechecked against the corrected documents. The AAOS hip OA guideline (eight recommendations and nine options for adults with diagnosed hip OA) was not previously profiled on this page (Hannon 2024, PMID 38781351).

OARSI voting produced explicit levels (1A through 5) after Core Treatments were locked; topical NSAIDs are Level 1A for knee OA, while oral NSAIDs are not recommended in cardiovascular comorbidity or frailty (Bannuru 2019, PMID 31278997). ACR/AF used GRADE strong/conditional for or against (Kolasinski 2020, PMID 31908149). NICE uses “offer / consider / do not offer” rather than GRADE class I–III. ESCEO 2019 is a GRADE-informed algorithm (strong and weak recommendations across 15 interventions) that remains more permissive of background SYSADOA than NICE (Bruyère 2019, PMID 31126594). EULAR 2023 (published 2024) updated Fernandes 2013 with two overarching principles and eight recommendations; agreement on the 2013 set was 8.0–9.1/10 (Moseng 2024, PMID 38212040; Fernandes 2013, PMID 23595142). EULAR hand OA 2018 prioritizes topical over systemic treatment (Kloppenburg 2019, PMID 30154087).

The disagreements persist because panels use different minimum-important-benefit thresholds, cost-effectiveness, product-heterogeneity tolerance and evidence cut-off dates — not because they reviewed disjoint literatures (Bichsel 2022, PMID 34411512).
