Corticosteroid: short-term symptom relief vs structure with repetition

Cochrane (27 trials, 1,767 people) judged evidence low quality for intra-articular corticosteroid vs sham because of inconsistency, imprecision and bias; benefits are short (Jüni 2015, PMID 26490760). Individual-patient OA Trial Bank analysis found greater short-term benefit in those with more severe baseline pain (van Middelkoop 2016, PMID 26836288). McAlindon randomized 140 people with ultrasound synovitis to 40 mg triamcinolone every 12 weeks for 2 years vs saline: cartilage-thickness loss −0.21 vs −0.10 mm (difference −0.11 mm, 95% CI −0.20 to −0.03) and no pain advantage (−0.6/20, −1.6 to 0.3) (McAlindon 2017, PMID 28510679). That is a scheduled-repetition finding, not a ban on a single injection. Extended-release microsphere TA (FX006 32 mg) prolonged synovial-fluid residence through week 12 versus crystalline 40 mg, whose synovial fluid was often unquantifiable by week 6, with much lower peak plasma TA (Kraus 2018, PMID 29024802). NICE: consider corticosteroid when other drugs fail, with an explicit 2–10 week relief expectation; do not offer hyaluronan (NG226).

Hyaluronan: small-study positive, large-trial near-null

Rutjes 2012 (89 trials, 12,667 adults): overall pain ES −0.37 (−0.46 to −0.28) with funnel-plot asymmetry; 18 large blinded trials ES −0.11 (−0.18 to −0.04); five unpublished trials ES −0.03 (−0.14 to 0.09) (Rutjes 2012, PMID 22868835). Pereira 2022 restricted to ≥100/arm placebo-controlled trials (24 trials, 8,997 people): pain SMD −0.08 (−0.15 to −0.02), about −2.0 mm on 100-mm VAS; serious AE RR 1.49 (1.12–1.98) (Pereira 2022, PMID 36333100). Prespecified MCID was SMD −0.37. ACR/AF conditionally against knee hyaluronate, strongly against hip; NICE does not offer it; some expert algorithms remain permissive — a threshold and product-heterogeneity dispute, not two evidence bases.

PRP: RESTORE vs product-heterogeneous meta-analysis

RESTORE (n=288, KL 2–3 medial OA): three weekly leukocyte-poor PRP injections vs saline; 12-month pain −2.1 vs −1.8 (difference −0.4/10, −0.9 to 0.2; MCID 1.8); cartilage-volume difference −0.2% (−1.9 to 1.5); 29/31 secondary outcomes null (Bennell 2021, PMID 34812863). Tang 2020 PRP-vs-HA meta-analysis favors PRP and is the literature most often quoted against RESTORE (Tang 2020, PMID 32912243). Hip injection network: no active treatment significantly beat saline at 2–4 or 6 months, though all groups improved from baseline (Gazendam 2021, PMID 32829298). Cell products: Cochrane low-certainty possible small pain/function benefit; structure and important harms uncertain (Whittle 2025, PMID 40169165).
