Long-term (≥12 month) pharmacologic NMA of 47 RCTs / 22,037 people is the reminder that most OA drug evidence is short (Gregori 2018, PMID 30575881).
Cardiovascular and GI harms are the binding constraint¶
CNT individual-participant meta-analysis: coxibs major vascular events RR 1.37 (1.14–1.66); diclofenac 1.41 (1.12–1.78); naproxen 0.93 (0.69–1.27); about 3 extra major vascular events per 1,000 patient-years on coxib or diclofenac, one fatal (CNT Collaboration 2013, PMID 23726390). Trelle network: rofecoxib MI rate ratio 2.12 (1.26–3.56); ibuprofen stroke 3.36 (1.00–11.6); etoricoxib and diclofenac highest cardiovascular death (Trelle 2011, PMID 21224324). Real-world IPD: 1–7 days of any NSAID raised MI odds — celecoxib 1.24 (0.91–1.82), ibuprofen 1.48 (1.00–2.26), diclofenac 1.50 (1.06–2.04), naproxen 1.53 (1.07–2.33), rofecoxib 1.58 (1.07–2.17) — including naproxen, contrary to a common “naproxen is CV-safe” heuristic (Bally 2017, PMID 28487435).
Guideline conflict on paracetamol is a threshold dispute: ACR/AF conditional option vs NICE “do not routinely offer” vs ESCEO stepwise algorithm that still places background SYSADOA/paracetamol differently (Kolasinski 2020, PMID 31908149; Bruyère 2019, PMID 31126594; NICE NG226).
