Red flags and safety concerns¶
TL;DR — Safety assessment must separate urgent psychiatric or medical risk from routine ADHD symptoms. New episodic mania, psychosis, suicidality, severe substance intoxication/withdrawal, syncope/exertional chest symptoms or dangerous sleepiness require targeted evaluation rather than automatic stimulant escalation. Across observational studies, ADHD medications were not significantly associated with overall cardiovascular disease, but a modest increase—especially arrhythmia-related—could not be excluded (Zhang 2022, PMID 36416824). Misuse and diversion, appetite/growth effects, sleep change, blood pressure/pulse and rare psychiatric adverse events require active monitoring (Wilens 2008, PMID 18174822; Carucci 2021, PMID 33080250).

Cardiovascular evidence¶
Meta-analysis found no statistically significant overall association between ADHD medication and cardiovascular disease across ages, but confidence intervals did not exclude modest risk and evidence was thinner for females, pre-existing disease and long exposure (Zhang 2022, PMID 36416824). A pediatric/young-adult cohort likewise found no increased serious-event signal while retaining uncertainty around small relative increases (Cooper 2011, PMID 22043968).
Misuse and diversion¶
Systematic reviews document nonmedical use and diversion of prescribed stimulants, particularly where peer demand and immediate-release access are high (Wilens 2008, PMID 18174822; Forrest 2025, PMID 40698051). Risk management should avoid denying effective care solely because misuse exists at population level.
Growth, appetite and sleep¶
Methylphenidate-associated height and weight effects are small on average but warrant individual trajectory monitoring (Carucci 2021, PMID 33080250). Stimulants can delay sleep in some patients while effective symptom control can improve bedtime organization in others; within-person timing matters.
Suicide and psychiatric outcomes¶
Observational meta-analysis did not show increased suicide-attempt risk with ADHD medication and suggested lower risk in some within-person analyses, but confounding and indication remain important (Liu 2020, PMID 32875686). New suicidal or psychotic symptoms always require direct assessment rather than inference from population averages.
How to read the evidence¶
- Diagnostic case–control separation is not the same as accuracy among referred patients.
- Symptom change is not interchangeable with functional improvement.
- Short randomized trials estimate acute efficacy, not decades-long benefit or harm.
- Observational within-person and target-trial designs reduce some confounding but remain nonrandomized.
- Parent, teacher, clinician, self-report and objective tasks measure different vantage points.
- Statistical significance does not establish a patient-important effect.
- Population averages should not be converted into deterministic individual predictions.
- Absence of evidence in a subgroup is not evidence of identical effects.
Minimum standards for the next evidence generation¶
- Prespecify one primary outcome and its clinically important threshold.
- Report symptoms, function, quality of life and harms separately.
- Use clinically relevant comparators rather than only healthy controls.
- Retain and report participants who discontinue treatment.
- Test generalization across sex, age, ancestry, comorbidity and setting.
- Separate model development, internal validation and external validation.
- Declare medication exposure, concomitant care and rater blinding.
- Make null and contradictory results visible rather than averaging away design differences.
Recurring evidence gaps¶
- Females, older adults and people with complex comorbidity remain underrepresented.
- Functional outcomes are measured less consistently than core symptoms.
- Follow-up usually ends before major educational or occupational transitions.
- Comparator choice often makes effects look more or less specific than they are.
- Treatment discontinuation and switching weaken long-term causal contrasts.
- Independent replication is uncommon for biomarkers and digital interventions.
