Diagnosis and assessment

TL;DR — ADHD diagnosis is a clinical synthesis, not a positive score on a questionnaire, continuous-performance task, EEG, scan or blood test (Peterson 2024, PMID 38523599). Standardized ratings and multiple informants improve structure and cross-setting coverage, but accuracy changes with referral context and comparator group. In children, pooled rating-scale sensitivity was 0.72–0.83 and specificity 0.73–0.84 across four instruments, insufficient for stand-alone diagnosis (Chang 2016, PMID 26928969). Adult screening scales can perform well in selected calibration samples, yet positive screens still require developmental, impairment and differential assessment (Kessler 2005, PMID 15841682; Ustun 2017, PMID 28384801).

The diagnostic sequence

  1. Establish current symptoms with examples rather than labels.
  2. Establish clinically important impairment.
  3. Obtain evidence across more than one setting and from more than one source where feasible.
  4. Reconstruct developmental onset without treating retrospective recall as infallible.
  5. Assess sleep, substances, mood, anxiety, learning, autism, trauma and medical explanations.
  6. Document uncertainty and revisit when context changes (Wolraich 2019, PMID 31570648; Peterson 2024, PMID 38523599).

Informants and context

Disagreement between parent, teacher and self-report is not merely measurement error: demands, supports and observation windows differ. The clinical question is whether the full record demonstrates pervasive impairment, not whether every rater supplies the same number (Tahıllıoğlu 2021, PMID 33612071).

Adult assessment

Adult assessment must connect present impairment to a plausible childhood course while actively testing alternatives. The DSM-5 ASRS screener showed sensitivity 91.4%, specificity 96.0%, AUC 0.94 and positive predictive value 67.3% in prevalence-weighted population calibration, but specificity was 74.0% in the independent clinical sample (Ustun 2017, PMID 28384801). Those shifts illustrate spectrum and setting effects rather than failure of the instrument.

How to read the evidence

  • Diagnostic case–control separation is not the same as accuracy among referred patients.
  • Symptom change is not interchangeable with functional improvement.
  • Short randomized trials estimate acute efficacy, not decades-long benefit or harm.
  • Observational within-person and target-trial designs reduce some confounding but remain nonrandomized.
  • Parent, teacher, clinician, self-report and objective tasks measure different vantage points.
  • Statistical significance does not establish a patient-important effect.
  • Population averages should not be converted into deterministic individual predictions.
  • Absence of evidence in a subgroup is not evidence of identical effects.

Minimum standards for the next evidence generation

  1. Prespecify one primary outcome and its clinically important threshold.
  2. Report symptoms, function, quality of life and harms separately.
  3. Use clinically relevant comparators rather than only healthy controls.
  4. Retain and report participants who discontinue treatment.
  5. Test generalization across sex, age, ancestry, comorbidity and setting.
  6. Separate model development, internal validation and external validation.
  7. Declare medication exposure, concomitant care and rater blinding.
  8. Make null and contradictory results visible rather than averaging away design differences.

Recurring evidence gaps

  • Females, older adults and people with complex comorbidity remain underrepresented.
  • Functional outcomes are measured less consistently than core symptoms.
  • Follow-up usually ends before major educational or occupational transitions.
  • Comparator choice often makes effects look more or less specific than they are.
  • Treatment discontinuation and switching weaken long-term causal contrasts.
  • Independent replication is uncommon for biomarkers and digital interventions.
