Psychosocial and digital treatment

TL;DR — Psychosocial interventions target parenting, classroom contingencies, organization, coping and functional goals that medication does not automatically normalize. Adult CBT meta-analysis supports improvement in core and emotional symptoms, usually as an adjunct or structured alternative (Liu 2023, PMID 36794797). The STARS-ADHD digital trial randomized 348 children and improved its objective attention endpoint by 0.93 versus 0.03 units, with uncertain translation to everyday function (Kollins 2020, PMID 33334505). Digital delivery can expand access, but engagement, comparator quality, blinding and durable functional outcomes determine whether it adds value.

Nonpharmacologic meta-analysis is sensitive to who rates the outcome and whether the rater is blinded; unblinded proximal ratings can overstate effects (Sonuga-Barke 2013, PMID 23360949).

CBT and skills-based care

Adult CBT-based interventions improve core and emotional symptoms in meta-analysis, but protocols, comparators and medication background vary (Liu 2023, PMID 36794797). Skills should be linked to an observable target rather than delivered as generic psychoeducation.

Digital therapeutics

STARS-ADHD assigned 180 children to AKL-T01 and 168 to digital control for four weeks. Mean TOVA Attention Performance Index change was 0.93 versus 0.03; median between-group estimate was 0.88 (95% CI 0.24–1.49). No serious adverse events occurred; frustration affected 3% and headache 2% of the intervention group (Kollins 2020, PMID 33334505; NCT02674633).

The objective endpoint demonstrates target engagement, not by itself clinically important symptom or school improvement. Independent replication, active comparators and follow-up are central.

Combination with medication

The MTA found combined care did not beat carefully managed medication for core symptoms directly but improved selected non-core domains (MTA Cooperative Group 1999, PMID 10591283). Combination value should therefore be judged by the domain it is intended to change.

How to read the evidence

  • Diagnostic case–control separation is not the same as accuracy among referred patients.
  • Symptom change is not interchangeable with functional improvement.
  • Short randomized trials estimate acute efficacy, not decades-long benefit or harm.
  • Observational within-person and target-trial designs reduce some confounding but remain nonrandomized.
  • Parent, teacher, clinician, self-report and objective tasks measure different vantage points.
  • Statistical significance does not establish a patient-important effect.
  • Population averages should not be converted into deterministic individual predictions.
  • Absence of evidence in a subgroup is not evidence of identical effects.

Minimum standards for the next evidence generation

  1. Prespecify one primary outcome and its clinically important threshold.
  2. Report symptoms, function, quality of life and harms separately.
  3. Use clinically relevant comparators rather than only healthy controls.
  4. Retain and report participants who discontinue treatment.
  5. Test generalization across sex, age, ancestry, comorbidity and setting.
  6. Separate model development, internal validation and external validation.
  7. Declare medication exposure, concomitant care and rater blinding.
  8. Make null and contradictory results visible rather than averaging away design differences.

Recurring evidence gaps

  • Females, older adults and people with complex comorbidity remain underrepresented.
  • Functional outcomes are measured less consistently than core symptoms.
  • Follow-up usually ends before major educational or occupational transitions.
  • Comparator choice often makes effects look more or less specific than they are.
  • Treatment discontinuation and switching weaken long-term causal contrasts.
  • Independent replication is uncommon for biomarkers and digital interventions.
