Epidemiology and lifespan course

TL;DR — There is no method-free ADHD prevalence. A 2024 meta-analysis estimated 1.6% in register studies, 5.0% in surveys, 4.2% in one-stage clinical studies and 4.8% in two-stage clinical studies (Popit 2024, PMID 39381949). Adult estimates differ depending on whether childhood onset is required: 2.58% for persistent adult ADHD versus 6.76% for symptomatic adult ADHD in a global synthesis (Song 2021, PMID 33692893). Older follow-up meta-analysis estimated about 15% full-syndrome and 65% symptomatic persistence at age 25, showing that remission definitions dominate the result (Faraone 2006, PMID 16420712).

The higher clinical referral of combined presentation than inattentive presentation can coexist with inattentive presentation being more common in population data (Willcutt 2012, PMID 22976615). Diagnosed prevalence also reflects access, recognition and prescribing systems; it is not interchangeable with structured epidemiologic prevalence.

Course and persistence

Full symptom-count persistence is a stricter endpoint than residual symptoms, impairment, treatment need or quality of life. The 15% versus 65% contrast at age 25 is therefore not a contradiction but a denominator and definition problem (Faraone 2006, PMID 16420712).

The Dunedin birth cohort complicated a simple childhood-to-adult continuity model: 90% of adults meeting the study’s modified adult ADHD definition lacked childhood ADHD, but onset and cross-setting corroboration were deliberately treated as outcomes rather than required criteria (Moffitt 2015, PMID 25998281). This provocative result does not erase the onset criterion; it exposes the need to distinguish false-negative childhood ascertainment, adult mimics and potentially different adult-onset pathways.

A post-2020 systematic review found no convincing increase in underlying prevalence in its highest-quality evidence, despite changing service demand and incidence around the pandemic; only 4 of 40 included studies were at low risk of bias (Martin 2025, PMID 40393551).

How to read the evidence

  • Diagnostic case–control separation is not the same as accuracy among referred patients.
  • Symptom change is not interchangeable with functional improvement.
  • Short randomized trials estimate acute efficacy, not decades-long benefit or harm.
  • Observational within-person and target-trial designs reduce some confounding but remain nonrandomized.
  • Parent, teacher, clinician, self-report and objective tasks measure different vantage points.
  • Statistical significance does not establish a patient-important effect.
  • Population averages should not be converted into deterministic individual predictions.
  • Absence of evidence in a subgroup is not evidence of identical effects.

Minimum standards for the next evidence generation

  1. Prespecify one primary outcome and its clinically important threshold.
  2. Report symptoms, function, quality of life and harms separately.
  3. Use clinically relevant comparators rather than only healthy controls.
  4. Retain and report participants who discontinue treatment.
  5. Test generalization across sex, age, ancestry, comorbidity and setting.
  6. Separate model development, internal validation and external validation.
  7. Declare medication exposure, concomitant care and rater blinding.
  8. Make null and contradictory results visible rather than averaging away design differences.

Recurring evidence gaps

  • Females, older adults and people with complex comorbidity remain underrepresented.
  • Functional outcomes are measured less consistently than core symptoms.
  • Follow-up usually ends before major educational or occupational transitions.
  • Comparator choice often makes effects look more or less specific than they are.
  • Treatment discontinuation and switching weaken long-term causal contrasts.
  • Independent replication is uncommon for biomarkers and digital interventions.
