Long-term outcomes and quality of life

TL;DR — ADHD is associated with educational, occupational, relationship, injury, psychiatric and mortality burdens, but association sizes reflect comorbidity, access and social context as well as core symptoms (Faraone 2021, PMID 33549739). Acute symptom improvement cannot be assumed to produce durable life-course change. A Swedish target-trial emulation of 148,578 people found medication initiation associated with lower 2-year all-cause mortality (HR 0.79, 95% CI 0.70–0.88), driven by unnatural causes; residual confounding remains possible (Li 2024, PMID 38470385). Quality of life should be measured directly rather than inferred from symptom scales (Bellato 2025, PMID 38823477).

Mortality and injury

In Sweden, 84,204 of 148,578 newly diagnosed people initiated medication. Two-year mortality risk was 39.1 versus 48.1 per 10,000; all-cause HR 0.79 (95% CI 0.70–0.88), unnatural-cause HR 0.75 (0.66–0.86), and natural-cause HR 0.86 (0.71–1.05) (Li 2024, PMID 38470385). The target-trial framework strengthens but does not convert observational data into randomization.

Pharmacologic treatment is associated with lower physical-injury risk in observational synthesis, but within-person and confounding controls determine credibility (Man 2017, PMID 29255995).

Education, work and relationships

Core symptoms interact with learning, socioeconomic resources, school supports, comorbidity and stigma. Studies should distinguish attainment from performance and unemployment from job fit.

Quality of life

Medication meta-analysis finds quality-of-life improvement on average, but changes are smaller and less consistently studied than symptom outcomes (Bellato 2025, PMID 38823477). Patient-important outcomes should be prespecified and followed after acute trials.

Causal limits of long follow-up

MTA follow-up illustrates treatment crossover and selection: once randomized strategies stop being distinct, later group differences cannot be interpreted as preserved randomized effects (Swanson 2008, PMID 18573923).

How to read the evidence

  • Diagnostic case–control separation is not the same as accuracy among referred patients.
  • Symptom change is not interchangeable with functional improvement.
  • Short randomized trials estimate acute efficacy, not decades-long benefit or harm.
  • Observational within-person and target-trial designs reduce some confounding but remain nonrandomized.
  • Parent, teacher, clinician, self-report and objective tasks measure different vantage points.
  • Statistical significance does not establish a patient-important effect.
  • Population averages should not be converted into deterministic individual predictions.
  • Absence of evidence in a subgroup is not evidence of identical effects.

Minimum standards for the next evidence generation

  1. Prespecify one primary outcome and its clinically important threshold.
  2. Report symptoms, function, quality of life and harms separately.
  3. Use clinically relevant comparators rather than only healthy controls.
  4. Retain and report participants who discontinue treatment.
  5. Test generalization across sex, age, ancestry, comorbidity and setting.
  6. Separate model development, internal validation and external validation.
  7. Declare medication exposure, concomitant care and rater blinding.
  8. Make null and contradictory results visible rather than averaging away design differences.

Recurring evidence gaps

  • Females, older adults and people with complex comorbidity remain underrepresented.
  • Functional outcomes are measured less consistently than core symptoms.
  • Follow-up usually ends before major educational or occupational transitions.
  • Comparator choice often makes effects look more or less specific than they are.
  • Treatment discontinuation and switching weaken long-term causal contrasts.
  • Independent replication is uncommon for biomarkers and digital interventions.
