History, nosology and presentations

TL;DR — ADHD terminology has changed repeatedly, but the core clinical problem—developmentally atypical inattention and/or hyperactivity-impulsivity with impairment—has remained recognizable (Leffa 2022, PMID 35397064). “ADD” is obsolete terminology, not a separate current disorder. Current presentations are useful descriptions but show longitudinal movement and do not define discrete biological diseases (Wagner 2016, PMID 25877403; Overgaard 2023, PMID 35737107). Dimensional evidence and the first GWAS support ADHD as an extreme of continuously distributed heritable traits while leaving categorical diagnosis useful for clinical decisions (Demontis 2019, PMID 30478444).

From hyperactivity to attention and development

The diagnostic label broadened as clinical descriptions incorporated attention regulation, impulsivity, developmental course and adult persistence. DSM-era changes should not be interpreted as discovery of successive diseases; each revision changed thresholds, examples and presentation structure (Leffa 2022, PMID 35397064).

Categories versus dimensions

Twin meta-analysis estimated genetic contributions of 71% for inattention and 73% for hyperactivity-impulsivity, with partly different genetic architecture (Nikolas 2010, PMID 20141238). GWAS concordance between diagnosis and population symptom measures supports a dimensional liability model (Demontis 2019, PMID 30478444). A threshold remains operationally necessary for access, treatment and research eligibility, but it is not a biological cliff.

Presentation stability

Parent and teacher descriptions can diverge because behavior is context-dependent and informants observe different demands. Longitudinal preschool-to-school-age work finds both stability and change, reinforcing that current presentation should be re-evaluated rather than copied indefinitely (Overgaard 2023, PMID 35737107; Bunte 2014, PMID 24781411).

Adult symptom expression often shifts away from overt running or climbing toward restlessness, disorganization, time-management failure and impulsive decisions (Weibel 2020, PMID 31610922). Evidence for symptom stability in older adults exists, but late-life diagnosis remains an underdeveloped evidence area (Semeijn 2016, PMID 26068984).

How to read the evidence

  • Diagnostic case–control separation is not the same as accuracy among referred patients.
  • Symptom change is not interchangeable with functional improvement.
  • Short randomized trials estimate acute efficacy, not decades-long benefit or harm.
  • Observational within-person and target-trial designs reduce some confounding but remain nonrandomized.
  • Parent, teacher, clinician, self-report and objective tasks measure different vantage points.
  • Statistical significance does not establish a patient-important effect.
  • Population averages should not be converted into deterministic individual predictions.
  • Absence of evidence in a subgroup is not evidence of identical effects.

Minimum standards for the next evidence generation

  1. Prespecify one primary outcome and its clinically important threshold.
  2. Report symptoms, function, quality of life and harms separately.
  3. Use clinically relevant comparators rather than only healthy controls.
  4. Retain and report participants who discontinue treatment.
  5. Test generalization across sex, age, ancestry, comorbidity and setting.
  6. Separate model development, internal validation and external validation.
  7. Declare medication exposure, concomitant care and rater blinding.
  8. Make null and contradictory results visible rather than averaging away design differences.

Recurring evidence gaps

  • Females, older adults and people with complex comorbidity remain underrepresented.
  • Functional outcomes are measured less consistently than core symptoms.
  • Follow-up usually ends before major educational or occupational transitions.
  • Comparator choice often makes effects look more or less specific than they are.
  • Treatment discontinuation and switching weaken long-term causal contrasts.
  • Independent replication is uncommon for biomarkers and digital interventions.
