Adult ADHD¶
TL;DR — Adult ADHD requires evidence of a developmental disorder, current impairment and exclusion of better explanations; adult symptoms often appear as disorganization, internal restlessness and impulsive decisions rather than childhood stereotypes (Rivas-Vazquez 2023, PMID 36743427). Global synthesis estimated 2.58% persistent adult ADHD and 6.76% symptomatic adult ADHD, demonstrating the effect of the childhood-onset rule (Song 2021, PMID 33692893). Medication and CBT improve short-term symptoms on average, but occupational, relationship and long-term quality-of-life evidence is thinner (Elliott 2020, PMID 33085721; Liu 2023, PMID 36794797).
Recognition without lowering specificity¶
Adult assessment should reconstruct school-age behavior and impairment, seek collateral or records when available, and test competing explanations including mood episodes, anxiety, trauma, sleep disorders, substance effects and cognitive disorders. A positive ASRS is an invitation to assess, not a diagnosis (Kessler 2005, PMID 15841682).

Adult-onset controversy¶
In the Dunedin cohort, childhood and adult ADHD groups were almost nonoverlapping and 90% of study-defined adult cases lacked childhood ADHD. Because the adult definition omitted onset and cross-setting corroboration by design, the finding supports careful etiologic investigation rather than automatic removal of the onset criterion (Moffitt 2015, PMID 25998281).
Treatment evidence¶
Network meta-analysis supports several medications for short-term symptom reduction but highlights limitations in longer-duration evidence and patient-priority outcomes (Elliott 2020, PMID 33085721). CBT-based interventions improve adult core and emotional symptoms in meta-analysis and can address organization, avoidance and compensatory habits (Liu 2023, PMID 36794797).
Later life¶
Older-adult data are sparse. Symptom stability has been observed, but sensory change, sleep, medical illness, polypharmacy and cognitive decline expand the differential and safety burden (Semeijn 2016, PMID 26068984).
How to read the evidence¶
- Diagnostic case–control separation is not the same as accuracy among referred patients.
- Symptom change is not interchangeable with functional improvement.
- Short randomized trials estimate acute efficacy, not decades-long benefit or harm.
- Observational within-person and target-trial designs reduce some confounding but remain nonrandomized.
- Parent, teacher, clinician, self-report and objective tasks measure different vantage points.
- Statistical significance does not establish a patient-important effect.
- Population averages should not be converted into deterministic individual predictions.
- Absence of evidence in a subgroup is not evidence of identical effects.
Minimum standards for the next evidence generation¶
- Prespecify one primary outcome and its clinically important threshold.
- Report symptoms, function, quality of life and harms separately.
- Use clinically relevant comparators rather than only healthy controls.
- Retain and report participants who discontinue treatment.
- Test generalization across sex, age, ancestry, comorbidity and setting.
- Separate model development, internal validation and external validation.
- Declare medication exposure, concomitant care and rater blinding.
- Make null and contradictory results visible rather than averaging away design differences.
Recurring evidence gaps¶
- Females, older adults and people with complex comorbidity remain underrepresented.
- Functional outcomes are measured less consistently than core symptoms.
- Follow-up usually ends before major educational or occupational transitions.
- Comparator choice often makes effects look more or less specific than they are.
- Treatment discontinuation and switching weaken long-term causal contrasts.
- Independent replication is uncommon for biomarkers and digital interventions.
