Sleep is both outcome and amplifier

Painful diabetic neuropathy interviews identified sleep initiation, nocturnal waking and non-restorative sleep; zoster/PHN interviews found sleep affected in 7/10 participants (PMID 25354872; PMID 39232368).

Mobility includes fear and safety

Walking, standing, balance and exercise limitations appeared in the 70-person diabetic study; SCI synthesis links neuropathic pain with broad life impact (PMID 25354872; PMID 28451808).

Work and ordinary roles are core outcomes

Productivity, concentration, chores and recreation were reported in diabetic neuropathic pain, while PHN affected daily living in 9/10 participants in the Argentina sample (PMID 25354872; PMID 39232368).

Treatment burden accumulates

Only minorities achieve large benefit from any one medicine, while dizziness, somnolence and discontinuation recur across gabapentinoid trials (PMID 25575710; PMID 28597471; PMID 30673120).

Diagnostic acknowledgment matters

Symptoms are real even when lesion certainty remains possible rather than definite. The grading framework enables transparent uncertainty rather than dismissal (PMID 27115670; PMID 37253688).
