Clinical practice guidelines

TL;DR — Guidelines agree that symptomatic diagnosis requires clinical–imaging concordance and that urgent neurological deterioration changes the pathway. The 2021 nonoperative guideline supports multimodal education/exercise/manual care and recommends against routine epidural steroid injection for lumbar neurogenic claudication (Bussières 2021, PMID 33857615). DCM guidance recommends surgery for moderate/severe disease, surgery or structured rehabilitation for mild disease, and no prophylactic surgery for compression alone without radiculopathy (Fehlings 2017, PMID 29164035). Lumbar surgical recommendations are older and often rest on lower-certainty comparative evidence.

NASS lumbar guideline

The NASS evidence-based lumbar-stenosis guideline updated diagnostic and treatment recommendations and remains a major North American reference (Kreiner 2013, PMID 23830297). Its age matters because it predates several decompression-versus-fusion trials and contemporary rehabilitation syntheses.

Nonoperative guideline

A multidisciplinary guideline for neurogenic claudication recommends education, exercise, manual therapy and rehabilitation within multimodal care and recommends against epidural steroid injection (Bussières 2021, PMID 33857615). Recommendation strength reflects both modest benefit and low direct harm.

WFNS recommendations

WFNS committees separated natural course/diagnosis from decompression and fusion questions (Zileli 2020, PMID 32613187) (Costa 2020, PMID 32613189), with a parallel module on conservative care and percutaneous pain-relief techniques (Fornari 2020, PMID 32613192). This helps prevent imaging definitions from silently becoming procedural indications.

Later lumbar consensus documents

MIST 2.0 addresses minimally invasive lumbar-stenosis procedures (Deer 2022, PMID 35546905), while an international Delphi study proposed a standardized stepped treatment pathway (Comer 2022, PMID 35676677). The 2025 ISSLS taskforce instead standardizes clinical outcome measures (Anderson 2025, PMID 40924201); measurement consensus must not be mistaken for evidence that a treatment is effective.

DCM 2017 guideline

The AO Spine/CSRS guideline strongly recommends surgery for moderate and severe DCM; suggests surgery or supervised structured rehabilitation for mild disease; and recommends surgery if deterioration occurs (Fehlings 2017, PMID 29164035). Nonmyelopathic compression without radiculopathy should be educated and followed, not prophylactically operated.

Updated DCM recommendations

AO Spine's later evidence review retains the central 2017 structure while integrating newer mild-disease, timing and imaging evidence (Fehlings 2025, PMID 40257837). Guideline updates should state which recommendations changed and which simply gained supporting literature.

Disagreements

Guidelines differ in how directly they discourage injections, what conservative package they endorse, how they define instability, and when lumbar fusion is justified. The DCM field has clearer severity-linked recommendations than lumbar stenosis because irreversible cord loss changes the risk balance.

Using guidelines

A recommendation is not a patient-specific prediction. Record phenotype, severity, trajectory, concordant imaging, prior care, frailty, objectives and preferences. When evidence is old or indirect, show that uncertainty rather than converting consensus into a numeric guarantee.

Interpretation rules

  • Anatomical narrowing is necessary but not sufficient for a symptomatic stenosis diagnosis.
  • Lumbar claudication, lumbar radiculopathy, cervical myelopathy and imaging-only compression are analyzed separately.
  • Absolute changes, confidence intervals and responder proportions outrank within-group significance.
  • Comparator choice determines the question a trial can answer.
  • Reoperation, falls, delirium and recovery burden are outcomes, not footnotes.
  • Subgroup claims require an interaction or externally validated treatment-effect model.
  • Older age is not a contraindication; reserve, goals and modifiable vulnerability are assessed directly.
  • Absence of deterioration in a cohort does not guarantee individual stability.
  • Evidence is research knowledge, not individualized medical advice.
