Cervical decompression and fusion¶
TL;DR — Cervical surgery aims to stop ongoing cord injury and create neurological recovery opportunity; complete normalization is not guaranteed. Approach is driven by compression direction, number of levels, alignment, instability, axial pain and patient risk—not a universal anterior/posterior hierarchy (Kato 2018, PMID 30279123). Laminoplasty preserves motion; laminectomy plus fusion controls alignment; anterior decompression directly addresses focal ventral disease. Comparative evidence is largely observational or heterogeneous meta-analysis (Xu 2017, PMID 28687344) (Zhao 2021, PMID 35111804).
Goals and timing¶
For moderate/severe DCM, guidelines recommend surgery; mild disease may undergo surgery or monitored structured rehabilitation, with surgery for deterioration (Fehlings 2017, PMID 29164035). The operation removes compression but does not erase established cord injury.
Anterior strategies¶
ACDF and corpectomy provide direct ventral decompression, restore segmental lordosis and fuse treated levels. Multilevel exposure increases dysphagia, pseudarthrosis and adjacent-segment concerns; anatomy and alignment determine feasibility (Kato 2018, PMID 30279123).
Posterior strategies¶
Laminoplasty expands the canal while preserving motion and is generally suited to multilevel disease with maintained lordosis. Laminectomy plus fusion adds stability and alignment control but increases construct-related burden (Weinberg 2020, PMID 32309667) (Abduljabbar 2018, PMID 29173440).
Comparative outcomes¶
Meta-analyses comparing multilevel ACDF with laminoplasty and laminoplasty with laminectomy/fusion show tradeoffs rather than a stable winner (Xu 2017, PMID 28687344) (Phan 2017, PMID 27342611) (Zhao 2021, PMID 35111804). Confounding by alignment and disease extent remains substantial.
Complications¶
Anterior harms include dysphagia, recurrent laryngeal injury, graft/implant failure and pseudarthrosis; posterior harms include C5 palsy, axial pain, wound problems and loss of motion when fused. Recent comparative complication syntheses reinforce approach-specific profiles (Hirai 2025, PMID 40838087).
Recovery measurement¶
mJOA, Nurick, dexterity, gait, quality of life and falls capture different consequences. Patient-reported improvements can occur with either approach when selection is appropriate (Lambrechts 2023, PMID 36716386); neurological stabilization is itself a meaningful outcome.
Adjuncts¶
Neuroprotection remains investigational. A secondary analysis of CSM-PROTECT did not establish riluzole as routine care (Fehlings 2024, PMID 38904964). Rehabilitation after surgery is plausible but trial evidence is still developing.

Interpretation rules¶
- Anatomical narrowing is necessary but not sufficient for a symptomatic stenosis diagnosis.
- Lumbar claudication, lumbar radiculopathy, cervical myelopathy and imaging-only compression are analyzed separately.
- Absolute changes, confidence intervals and responder proportions outrank within-group significance.
- Comparator choice determines the question a trial can answer.
- Reoperation, falls, delirium and recovery burden are outcomes, not footnotes.
- Subgroup claims require an interaction or externally validated treatment-effect model.
- Older age is not a contraindication; reserve, goals and modifiable vulnerability are assessed directly.
- Absence of deterioration in a cohort does not guarantee individual stability.
- Evidence is research knowledge, not individualized medical advice.
