Cervical stenosis and degenerative myelopathy¶
TL;DR — Degenerative cervical myelopathy (DCM) is spinal-cord dysfunction caused by age-related cervical compression, not merely a narrow canal on MRI. Hand clumsiness, gait imbalance, hyperreflexia, pathological reflexes and sphincter change form the clinical pattern (Kane 2020, PMID 33320508). Mild, moderate and severe disease correspond to mJOA 15–17, 12–14 and 0–11 (Tetreault 2017, PMID 27342612). Surgery is recommended for moderate/severe DCM and for deterioration; mild stable disease may enter surgery or closely monitored structured rehabilitation (Fehlings 2017, PMID 29164035).
Clinical syndrome¶
DCM combines upper-motor-neuron signs below the lesion with segmental root or lower-motor-neuron findings at the lesion. Early complaints include loss of dexterity, dropping objects, altered handwriting, imbalance and limb paresthesias; neck pain may be absent (Williams 2022, PMID 36208893).
Examination¶
Gait observation, tandem walking, hand opening–closing, grip-release, reflexes, Hoffmann/Babinski responses, clonus, strength and sensory testing are complementary. No single sign rules disease in or out; clusters and longitudinal change matter (Balmaceno-Criss 2024, PMID 39685599).
Severity¶
In 757 patients, ROC anchoring to Nurick grade supported mJOA 15–17 as mild, 12–14 as moderate and 0–11 as severe; quality of life and impairment differed across strata (Tetreault 2017, PMID 27342612). A ceiling effect remains possible in early disease, especially when dexterity or balance matters more than gross independence.
Natural history¶
Modeled neurologic stability declines over time: mild disease 91% at one year and 75% at five years; moderate/severe disease 83% and 55%, respectively (Sarraj 2024, PMID 37549831). Wide uncertainty and selection effects prevent a precise personal countdown, but deterioration cannot be assumed benign.
Falls¶
Among 135 surgical candidates, 64 fell at least once through one year; lower grip strength and potassium were independent predictors (Inose 2021, PMID 33496544). In a separate prospective diary study, falls fell from 497.4 to 90.3 per 100 person-years after surgery and motor deterioration per fall from 34% to 8% (Kimura 2020, PMID 31809466).
Compression without myelopathy¶
Nonmyelopathic compression should prompt education and follow-up, not automatic prophylactic surgery. Coexisting radiculopathy increases risk of developing myelopathy and can justify closer surveillance or operative discussion (Fehlings 2017, PMID 29164035).
Timing¶
Longer symptom duration and greater baseline impairment reduce the ceiling for recovery, although meaningful improvement remains possible. Diagnostic delay is common because symptoms overlap with peripheral neuropathy, carpal tunnel syndrome, stroke, multiple sclerosis and normal aging (Milligan 2019, PMID 31515310). Recognition is therefore a safety intervention.

Interpretation rules¶
- Anatomical narrowing is necessary but not sufficient for a symptomatic stenosis diagnosis.
- Lumbar claudication, lumbar radiculopathy, cervical myelopathy and imaging-only compression are analyzed separately.
- Absolute changes, confidence intervals and responder proportions outrank within-group significance.
- Comparator choice determines the question a trial can answer.
- Reoperation, falls, delirium and recovery burden are outcomes, not footnotes.
- Subgroup claims require an interaction or externally validated treatment-effect model.
- Older age is not a contraindication; reserve, goals and modifiable vulnerability are assessed directly.
- Absence of deterioration in a cohort does not guarantee individual stability.
- Evidence is research knowledge, not individualized medical advice.
