Diagnosis and differential diagnosis

TL;DR — Spinal stenosis is a clinicoradiological diagnosis: symptoms and signs identify a neural syndrome; imaging identifies a plausible anatomical substrate. Individual lumbar history or examination findings have limited standalone accuracy (Cook 2020, PMID 31312914). Neurogenic and vascular claudication, hip osteoarthritis, peripheral neuropathy and radiculopathy often overlap. For cervical disease, failure to examine gait and upper-motor-neuron signs is a common route to delay (Milligan 2019, PMID 31515310).

Diagnostic sequence

First define the syndrome—claudication, radiculopathy, myelopathy or acute cauda-equina dysfunction—then localize, image, and test discordant alternatives. Starting with the scan invites attribution of common incidental narrowing (Katz 2022, PMID 35503342).

Lumbar history

Posture-dependent leg or buttock symptoms, improvement with sitting/flexion and reduced walking tolerance support neurogenic claudication (Genevay 2010, PMID 20227646). Back pain alone is insufficient; a fixed dermatomal syndrome may be radiculopathy rather than claudication.

Lumbar examination

Strength, reflexes, sensation, nerve tension, pulses, capillary refill, hip motion and gait address different hypotheses. A normal resting neurological examination does not exclude dynamic claudication, while reduced pulses or exertional calf symptoms relieved by standing suggest vascular testing (Cook 2020, PMID 31312914).

Cervical examination

Hand dexterity, grip-release, reflexes, Hoffmann and Babinski responses, clonus, proprioception, Romberg and tandem gait should be assessed together (Balmaceno-Criss 2024, PMID 39685599). Peripheral entrapment can coexist and does not explain leg hyperreflexia or a broad gait disorder.

Imaging strategy

MRI is preferred for neural elements and competing lesions; CT is stronger for bone and CT myelography is a selective alternative. Quantitative lumbar thresholds vary and do not establish symptomatic disease (Steurer 2011, PMID 21798008). Hip imaging or vascular studies follow the clinical competing hypothesis.

Key mimics

Hip–spine syndrome is common because hip osteoarthritis and stenosis share buttock/thigh pain and walking restriction (Devin 2012, PMID 22751162). Polyneuropathy, motor-neuron disease, multiple sclerosis, normal-pressure hydrocephalus, Parkinsonism and B12 deficiency can mimic parts of DCM; distribution and long-tract signs guide testing.

Urgency

New urinary retention or overflow, saddle anesthesia, bilateral progressive deficits or rapidly worsening weakness raises suspected cauda-equina syndrome (Kuris 2021, PMID 34473966). Red-flag frameworks are triage aids with imperfect accuracy, so a coherent evolving syndrome outweighs absence of one classic feature (Finucane 2020, PMID 32438853).

Interpretation rules

  • Anatomical narrowing is necessary but not sufficient for a symptomatic stenosis diagnosis.
  • Lumbar claudication, lumbar radiculopathy, cervical myelopathy and imaging-only compression are analyzed separately.
  • Absolute changes, confidence intervals and responder proportions outrank within-group significance.
  • Comparator choice determines the question a trial can answer.
  • Reoperation, falls, delirium and recovery burden are outcomes, not footnotes.
  • Subgroup claims require an interaction or externally validated treatment-effect model.
  • Older age is not a contraindication; reserve, goals and modifiable vulnerability are assessed directly.
  • Absence of deterioration in a cohort does not guarantee individual stability.
  • Evidence is research knowledge, not individualized medical advice.
