Conservative and rehabilitation care

TL;DR — For stable lumbar neurogenic claudication, the best-supported conservative package combines education, individualized exercise and often manual therapy rather than a single modality (Bussières 2021, PMID 33857615) (Ammendolia 2022, PMID 35046008). Across 13 exercise trials (1,440 participants), interventions used 60 components; cycling, strengthening, stretching and psychologically informed elements appeared more often in successful programs, but dose and supervision remain unresolved (Comer 2024, PMID 37715644). Medicines are adjuncts with condition-specific evidence weaker than for rehabilitation. Progressive DCM is not a routine exercise problem and requires surgical reassessment.

Who is suitable

Conservative care fits tolerable, stable lumbar symptoms without progressive deficit or cauda-equina features. Goals are more walking, less symptom interference and sustained activity rather than anatomical enlargement of the canal (Bussières 2021, PMID 33857615).

Education and self-management

Education explains posture sensitivity, safe activity progression, pacing and deterioration triggers. It should counter both fear-driven inactivity and indiscriminate pushing through neurological decline. Shared goals need an objective baseline such as walking time or steps.

Exercise ingredients

Most studied programs include flexion-tolerant conditioning and supervision; strengthening, stretching, cycling and psychologically informed approaches appeared more often in successful trials (Comer 2024, PMID 37715644). Balance was rarely included despite an older population, exposing a design gap.

Comparative trials

In 259 adults with mean age 72.4 years, manual therapy/individualized exercise was compared with group exercise and medical care (Schneider 2019, PMID 30646197). In 86 participants, supervised PT increased six-week responder proportions versus home exercise by 30.2 percentage points for Zurich symptom severity and 39.5 points for walking distance (Minetama 2019, PMID 30986577).

Medication evidence

Analgesics may address pain but do not reverse stenosis. A systematic review evaluated gabapentin/pregabalin with mixed efficacy and safety evidence (Martínez 2023, PMID 38094885); calcitonin reviews have not established a dependable routine benefit (Podichetty 2011, PMID 21325931) (Lu 2024, PMID 38436716). Older-adult sedation and fall risk matter.

Cervical boundary

For mild DCM, guidance permits either surgery or a supervised structured rehabilitation trial with close clinical monitoring; neurological deterioration should trigger operative evaluation (Fehlings 2017, PMID 29164035). Manipulation across a compressed cord is not interchangeable with lumbar mobilization.

Measuring response

Record walking distance/time, Zurich or ODI function, pain, participation and adverse events. A program can improve confidence or community activity without changing peak pain, and vice versa. Failure is persistent goal-level disability after adequate delivery, not failure to normalize MRI.

Interpretation rules

  • Anatomical narrowing is necessary but not sufficient for a symptomatic stenosis diagnosis.
  • Lumbar claudication, lumbar radiculopathy, cervical myelopathy and imaging-only compression are analyzed separately.
  • Absolute changes, confidence intervals and responder proportions outrank within-group significance.
  • Comparator choice determines the question a trial can answer.
  • Reoperation, falls, delirium and recovery burden are outcomes, not footnotes.
  • Subgroup claims require an interaction or externally validated treatment-effect model.
  • Older age is not a contraindication; reserve, goals and modifiable vulnerability are assessed directly.
  • Absence of deterioration in a cohort does not guarantee individual stability.
  • Evidence is research knowledge, not individualized medical advice.
