Battié et al. The Twin Spine Study: contributions to a changing view of disc degeneration¶
One-paragraph summary¶
This review integrates the exposure-discordant twin program launched in 1991. Across occupational loading, vibration, smoking, anthropometry, heritability and genetic analyses, familial influence was substantial while ordinary loading and vibration effects were smaller than the dominant injury model predicted (PMID 19111259). Smoking showed a small adverse effect, and some routine loading signals were compatible with benefit. The review shifted the field from a simple “wear-and-tear” narrative to interacting inherited and environmental determinants.
Key findings¶
- Large familial/genetic influence across lumbar-degeneration measures.
- Little within-pair effect despite large occupational and leisure-load discordance.
- No controlled evidence that motor-vehicle vibration accelerated degeneration.
- Smoking increased degeneration, but effect size was small.

Limitations¶
- Narrative integration rather than one prespecified meta-analysis.
- Mostly adult male Finnish/Canadian/US twin samples.
- Familial aggregation includes shared environment unless modeled separately.
- Exposure measurement relied partly on retrospective report.
- MRI degeneration is not equivalent to pain or disability.
Why it matters¶
The study program overturned a culturally powerful but overly simple causal model. It is the basis for communicating that occupation and injury can matter without explaining most population degeneration.
