CheckMate 017 — nivolumab versus docetaxel in advanced LUSC¶
One-paragraph summary¶
CheckMate 017 randomized 272 patients with advanced squamous NSCLC progressing during or after platinum chemotherapy to nivolumab or docetaxel. Nivolumab improved median overall survival (OS) from 6.0 to 9.2 months, reduced the hazard of death by 41%, improved response rate, and produced far fewer grade 3–4 treatment-related adverse events. PD-L1 expression was neither prognostic nor predictive in this trial.

Interpretation¶
The separation between modest median PFS and clearer OS/response benefit is characteristic of checkpoint trials with durable benefit in a subset. PD-L1 non-predictiveness in CheckMate 017 should not be generalized to every drug, line, assay, or regimen. The docetaxel comparator was appropriate for its era; most contemporary patients receive checkpoint therapy earlier.
Limitations¶
- Open-label design.
- ECOG 0–1 population; limited transportability to poor performance status.
- Checkpoint-naïve setting is now less common after first-line chemo-immunotherapy.
- Exclusions around autoimmune disease and other comorbidity limit generalization.
- PD-L1 assays and thresholds evolved after the trial.
Why it matters¶
This was the first decisive randomized survival improvement with a better toxicity profile in pretreated LUSC. It ended the idea that squamous disease was immunologically unresponsive and established a durable-benefit treatment class where targeted therapy had repeatedly failed.
