Development and validation of the FIQR, an online-administered revision of the 1991 Fibromyalgia Impact Questionnaire (Burckhardt 1991, PMID 1865419) built with a patient focus group to fix the FIQ's known deficiencies (dated function items, cumbersome scoring). The FIQR keeps the FIQ's three domains (function, overall impact, symptoms), scores every item 0–10, and adds items for memory, tenderness, balance, and environmental sensitivity. In 202 FM patients, 51 RA/SLE patients, 11 patients with major depressive disorder, and 213 healthy controls: mean total FIQR 56.6 ± 19.9 vs FIQ 60.6 ± 17.8; total-score correlation with FIQ r=0.88 (domain correlations 0.69–0.88); FIQR domains predicted all eight SF-36 subscales; discrimination between FM and each comparison group was good (healthy controls 12.1, RA/SLE 28.6, depression 17.3). Completion took 1.3 minutes; scoring ~1 minute.

Limitations

  • Online, self-selected validation samples (small MDD group, n=11); no test–retest or responsiveness data in the initial paper.
  • Severity/MCID anchors were derived in pregabalin-trial populations, not community samples.
  • Content predates the OMERACT core-set finalization; cognition is represented by a single item though patients rank it among their top problems.

Why it matters

The FIQR is the de facto standard disease-specific outcome of the modern trial era — primary or key secondary endpoint from tai chi trials (Wang 2018, PMID 29563100) to the TNX-102 SL phase 3 program (Lederman 2023, PMID 37165930) — and the instrument through which "multidimensional function," the OMERACT core domain, is operationalized. Its severity bands are also the leading candidates for operationalizing stepped-care thresholds.
